DISCHARGE PLANNING & THE MCHD RED TEAM
When you are ready to be discharged from the hospital, you will be visited by individuals on our "RED" team. They will discuss with you what to do when you arrive home to best help you until you are completely healed. They will also provide education in certain areas should you need it. Our RED team also has resources on other various educational classes available if they believe it may be useful to you and your health. If you need assistance with medications, transportation, or anything else that you need for your healing process after you leave MCHD, please either let your nurse or a member or the RED team know. They will contact our District Social Worker who has many community resources to help you after your stay should you need it.
Outpatient & Additional Services
MCHD has multiple outpatient services to help you after discharge. We offer Physical, Occupational, and Speech Therapies, Home Health and Hospice services, a Swing Bed Unit for patients who need additional care and therapy after admission, a sleep lab for those who are having sleep difficulties, and a 5-star nursing home for those who need full time assistance.
Our RED team can help to identify if your condition qualifies for additional outpatient care and can assist you in getting the services that you need.
